  • Abstract The activity that has the highest risk of workplace accidents is construction. In Indonesia, the greatest number of work accidents occur in this industry. The fabrication of offshore platforms is one of the construction activities. Scaffolding is a temporary structure that is necessary for all construction activities in order to support workers and materials. The majority of construction industry accidents are caused by working at heights, which includes using scaffolding. Therefore, to reduce the likelihood of these catastrophes, mitigating and preventive measures are required. Risk analysis is one way to accomplish this. According to the analysis’s findings, working with scaffolding carries 24 dangers, two of which are considered serious risks. Falling scaffolding materials and worker falls from heights are these two threats. The causes and effects of the two main dangers are determined using the bowtie diagram. Following that, the analysis results utilizing the fault tree and event tree methodologies indicate that there is a 4.581 x 10-7 risk of major injuries or death, 2.247 x 10-4 probability of mild injuries, and 8.036 x 10-7 probability of suspension trauma. There are 1.927 x 10-6 workers affected by the material, 1.639 x 10-6 property affected by the substance, and 4.294 x 10-4 material damage.
